Potential utility for following the activity of certain disease processes and the response to therapeutic interventions. However, elevations of CRP may be uncommon in the absence of traditional risk factors for CHD.48 CRP elevations may therefore be attributable to and already accounted for by traditional risk factors. A recent meta-analysis of studies of CRP and other circulating markers of inflammation in CHD risk prediction concluded that CRP may be a somewhat moderate predictor of CHD risk.49 These data tend to argue against the routine use of CRP measurements for clinical risk assessment in the general population. The Centers for Disease Control and Prevention and the AHA have published combined evidence-based guidelines for the optimal use of CRP in clinical practice.50 These guidelines confirm that, although hs-CRP measurement may be useful as an independent marker of prognosis for recurrent events, this analyte should not be used at present for routine screening, and application of secondary prevention measures should not be influenced by hs-CRP determination. However, in intermediate-risk patients without known CHD but with 10-year risk for CHD of 1020%, the guidelines suggest that measurement of hs-CRP may be considered an independent marker of CHD risk and may be useful, at the discretion of the physician, to help determine optimal treatment. Patients are categorized according to hs-CRP level as follows: 1 mg l low risk; 1.03.0 mg l average risk; and 3.0 mg l high risk. The report of the ATP III panel of the National Cholesterol Education Program also addressed the clinical use of CRP.29 This report does not recommend routine measurement of CRP for the purpose of modifying LDL cholesterol goals in primary prevention. However, because CRP may have predictive power beyond the classical lipid risk factors, this report indicates that some investigators consider lowering LDL cholesterol more aggressively in adults with elevated levels of CRP than may otherwise be.

Laws and policies emphasize that it is essential for opioid analgesics to be available for the treatment of moderate-to-severe pain and that prescribing should be individualized to the patient.2, 4-14 Although some progress has been made in treating pain, undertreatment of pain is still prevalent.15-17 Media coverage of diversion and abuse of controlled substances and uncertainty regarding potential disciplinary action may cause physicians to hesitate when considering treatment for a patient who may require long-term or high doses of opioids.1, 18 This is exacerbated when physicians have difficulty discerning between a patient with a legitimate pain problem and one who is feigning pain to obtain drugs for abuse or diversion.19 Because pain is subjective and cannot be measured or ruled out by laboratory tests or physical examination, physicians rely largely on their interpretation of patient interviews and histories to determine a patient's need for analgesics. However, they often find themselves in the predicament of wanting to treat seemingly legitimate patients without having information about their patients' prescription drug and medical histories that would help them identify and address any problems. Indeed, a 1999 report from the Institute of Medicine stressed that most medical errors do not result from individual practitioners' recklessness; rather, they are attributable to faulty processes and systems that lead people to make mistakes or fail to prevent them through lack of information and support in a complex working environment.20 Solving problems within healthcare requires the design of systems and processes to help and retrovir.

Competence and availability are the quiddity [essential qualities] of the good physician. A competent and available physician can do a great deal of good even if he is otherwise a despicable human being. An incompetent though saintly doctor is a great danger to the public. From these considerations it is easy to see why medical training has always had a semi-military character, hardly likely to be amenable to sensitivity-training. There is an iron bond involved. One is expected to extend oneself. One is also expected to exercise a certain authority over patients, since one must do, or have done, to them things that are painful.[178] Alan Gregg, described as one of the greatest medical educational spokesmen of his generation, disagrees. "The thing that really matters, " he wrote, "is not whether you do this or do that well. The thing that really matters is how much you care."[179] Patients tend to agree. One study, for example, found that most people consider bedside manner more important than even the technical skill of a physician.[180] Of course compassion and competence are not mutually exclusive, but medical training often places them at odds. From a prize winning essay entitled "The Self-Contradiction of a 'Humane' Profession": How does this supposedly humane and person oriented profession handle the all-important process of educating its professional offspring?. The answer is basically a cruel paradox: the powers that control medicine have evolved a process of education with the idea that only a tough, unrelenting, uncompassionate system can produce physicians truly capable of fulfilling its noble principles.[181] An article entitled "Humane Medicine Begins with Humane Medical Schools" notes that medical students, "have consistently denied or suppressed the emotional impact of disease in order to function within a system that rewards technical competence at the expense of personal development. If medical students are not cared for, they may learn not to care."[182] and reboxetine.

She tried topical treatment with triamcinolone cream, tretinoin, clobetasol, fluocinonide, pimecrolimus, and ammonium lactate creams with only slight relief of the itching. The diagnosis of syringomyelia, including the primary spinal form, is best established using MR imaging. The signal differences between watery CSF and neural tissue clearly outline the syrinx cavity on MR images. Administration of ionic contrast is helpful to define a tumor. Magnetic imaging, however, does not always clearly demonstrate the precise nature, level, or extent of the CSF obstruction. Cardiac-gaited techniques, used to study CSF flow properties at the craniocervical junction, have not been applied as successfully or widely to study other lev429.

Ing and bed linens. This may present a problem, particularly when it must be applied to the chest or back. ANTIBIOTICS Topical Antibiotics Topical antibiotics are useful in the treatment of mild to moderate inflammatory acne. They reduce the population of P acnes in sebaceous follicles and also demonstrate antiinflammatory properties by suppressing chemotaxis and decreasing the percentage of proinflammatory free fatty acids in surface lipids.9 Topical antibiotics available for use include erythromycin, clindamycin, sodium sulfacetamide, and salicylic acid. Erythromycin and clindamycin have been shown to be of equivalent efficacy for treating moderate acne.9, 14 They are often used effectively in combination with BPO or tretinoin. Sodium sulfacetamide is an antibacterial agent that has been used in antiacne preparations for many years. Until recently, it has only been available in combination with sulfur 5% as a keratolytic agent. Salicylic acid is effective against comedones and inflammatory lesions, but may be less effective in patients who cannot tolerate topical tretinoin. All topicals should be applied to the entire face rather than to individual lesions. All of the topical antibiotics can cause local irritation to some extent.6 Although more commonly associated with systemic clindamy.
