Hepatotoxicity Patients on nevirapine may experience an increase in liver enzymes and rarely severe hepatitis. After a full baseline LFT, ALT should be monitored regularly - after two weeks, four weeks and 2 months on nevirapine. Thereafter 3 monthly if no problems. Other drugs commonly used in HIV infected patients, notably anti-tuberculous therapy including prophylactic isoniazid ; may also cause hepatitis. Patients on PIs - especially indinavir - may develop unconjugated hyperbilirubinaemia resembling Gilbert's Syndrome. This generally does not require treatment.
Determination of urine NVP: Nrvirapine was tested in all the urine samples by a high performance liquid chromatography HPLC ; method described previously9. In brief, NVP was extracted into ethyl acetate. The organic layer was evaporated to dryness and the dried residue was reconstituted in the mobile phase and injected into the HPLC column. The HPLC system Shimadzu Corporation, Kyoto, Japan ; consisted of two pumps LC-10ATvp ; , diode array detector SPD-M 10AVP ; and system controller SCL-10AVP ; . The chromatogram was run for 20 min at a flow rate of 1.0 ml min at a column temperature of 40oC. Pure NVP solutions prepared in pooled urine were processed along with test samples in a similar manner and run every day. Under these chromatographic conditions, a retention time of 15 min was obtained for NVP. The presence of a visible peak around 15 min was considered to be positive for NVP. Results & Discussion Thirty six HIV-positive patients on treatment regimens with NVP were receiving antiretroviral treatment for a period ranging from 2 to 33 months. The time of urine collection after drug administration ranged from 20 min to 14 h. All the urine samples collected from these patients tested positive for NVP, as evidenced by a distinct peak at around 15 min. Standard NVP aided in identifying the peak, in the event of slight day-to-day variations in the retention time of NVP. The method for determination of urine NVP employed in this study was highly sensitive and was able to detect NVP concentrations as low as 0.01g ml. Although specificity of the method had been established by us, where interference due to co-administered drugs, namely, lamivudine, stavudine and zidovudine, was ruled out, this aspect and videx.
Feb. 13, 2003 Boston ; -- Cell-free HIV-1 RNA carrying the K103N mutation, the most important resistance mutation to nevirapine NVP ; , has been detected in the breast milk of at least 70% of women in the HPTN 023 trial in Zimbabwe. Higher concentrations of the mutation were found in breast milk than in plasma. Stanford University graduate student Esther Lee broke the news here on Feb. 12 at the 10th Conference on Retroviruses and Opportunistic Infections. She explained how plasma and breast milk were collected from the 33 women in the trial at 2, 8, 16, and 20 weeks. Both were assayed for viral load and resistance mutations to NVP. "There was a difference of more than two logs between plasma and breast milk viral load, " said Ms. Lee. Plasma samples had a median viral load of 4.6 log10 range, 2.6 log10 - 5.9 log10 breast milk samples had a median HIV RNA viral load of 2.5 log10 range, 0.9 log10 - 4.3 log10 ; , with little correlation between levels of the two in individual women. However, the frequency of NVP-resistant viral mutations was significantly higher in breast milk 65% ; than in plasma 24% ; . Ms. Lee said there did not seem to be a correlation between elevated viral load and the presence of resistant mutations. Finding the explanation for this differential selection of resistant virus in breast milk is the aim of ongoing research. Constance Benson, MD, was concerned by the study. The K103N mutation "confers cross-class resistance to all of the currently available NNRTIs." That could have a significant effect on the ability to offer effective regimens in resource-poor settings, specifically single-dose nevirapine administration at delivery. However, the former chair of the conference and a University of Colorado researcher cautioned against moving precipitously. Dr. Benson said, "We need to study it in a very careful way before making broad recommendations. I would not change policy at this point." Mark Harrington, from the Treatment Action Group, called single-dose nevirapine "the wedge by which treatment is being introduced into resourcepoor countries." He is afraid that opponents of treatment, particularly those within some African governments, will seize upon this study as yet another excuse for resisting the introduction or widespread use of anti-HIV treatment.

Efficacy of Antiretroviral Therapy Mother-to-child transmission MTCT ; may be prevented by treating the mother and or her baby with antiretroviral therapy ART ; . Studies have shown a 66% reduction or greater in vertical transmission with different antiretroviral treatment strategies, including both mono- and combination therapy. The first of these studies, initially reported in 1994, showed that the HIV-1 transmission rate for infants who received placebo was 22.6%, compared with 7.6% for those who received zidovudine AZT ; . This translates into a 66% reduction in risk for transmission in those who received treatment compared to those who received placebo. Other studies have shown similar results. One study compared nevirapine given to women at the onset of labor and infants at 48-72 hours of life with a very short regimen of AZT given orally during labor and to the infant for the first week of life. In this study, transmission of HIV among the infants at six weeks of age was 12% in the nevirapine arm verses 21% in the AZT arma reduction in transmission of nearly 50%. Another study reports transmission rates of 12.3% for nevirapine compared with 9.3% with AZT combined with lamivudine 3TC ; . In the US, the standard recommendation for infant prophylaxis is six weeks of AZT, regardless of whether the woman has had antepartum, intrapartum, or postpartum ART for details of AZT dosing in neonates, see perinatal guidelines website noted in the introduction to this article ; . It is unclear how intrapartum AZT prophylaxis works as maternal viral load suppression does not fully explain the observed efficacy. Nevirapine sustains advantage over
Zidovudine AZT ; , lamivudine 3TC ; and efavirenz Zidovudine lamivudine and stavudine lamivudine have emerged as the leading candidates for use in initial regimens because of their efficacy, toxicity profiles and years of clinical experience. AZT 3TC is favoured because of its marginally superior toxicity profile and lower pill burden when used as the fixed dose combination Combivir. Recent evidence has shown AZT 3TC regimens to produce better viral suppression than regimens that include didanosine ddI ; and stavudine d4T ; . The additive risk of peripheral neuropathy, hepatotoxicity and potential for lactic acidosis particularly in pregnant women ; when ddI and d4T are combined, plus the inconvenient dietary requirements when taking ddI, have also contributed to their fall from favour. d4T, among the NRTIs, has been most strongly linked to the development of lipoatrophy in some studies. The third drug is a choice between abacavir, efavirenz, nevirapine and lopinavir-ritonavir. The triple NRTI combination is probably less potent than NNRTI-containing regimens in those with a high viral load 55 000 ; , but evidence is lacking. The licensing of Trizivir, anticipated in South Africa in 2003, a fixed drug combination of AZT 3TC abacavir 1 capsule twice a day ; may tip the balance more in favour of this combination if the cost makes it feasible. Of the NNRTIs, efavirenz is probably superior to nevirapine in potency. A new formulation of efavirenz will reduce the pill burden to one tablet. Principal concerns surround the toxicity of efavirenz; teratogenicity, sleep disturbance and other CNS disorder. Risk of hepatitis in populations with significant levels of alcohol use, hepatitis B or concurrent anti-mycobacterial therapy reduce the suitability of nevirapine as a first-choice agent. Since efavirenz appears to have similar potency but a superior side-effect and drug interaction profile to the protease inhibitors, efavirenz is currently the drug of choice to complete the first-line regimen.
