A 36-year-old man with a history of hypertrophic obstructive cardiomyopathy presented to the emergency department complaining of severe, pleuritic chest pain radiating to his back. A diagnosis of hypertrophic obstructive cardiomyopathy had been made when the patient was a child. Because of symptoms of exertional dyspnea and chest pain refractory to medical therapy, he underwent implantation of a dual-chamber pacemaker in 1993 with the use of an atrial lead Accufix; Telectronics; Englewood, CO ; . The atrial lead is now known to be susceptible to fracture of the retention wire, leading to cardiac perforation or embolization J. W. At the time of surgery, the atrial lead was cut at the junction of the superior vena cava and right atrium as recommended, with the intention of removing the retention wire. Pharmacists in Suffolk have been offering advice to families as part of a campaign to improve the health of children. The two-week "Getting Suffolk fit for the future" project, which ran to the end of this week, involved young people aged five to 17 years being weighed and measured at pharmacies in the area. Pharmacists from 65 pharmacies provided information to parents and guardians about how to incorporate small changes in diet and activity into the family routines. Sheila Baldwin, a senior pharmacist at Suffolk Primary Care Trust, told The Journal that the PCT wanted to raise awareness of the health problems of young people remaining overweight later in life. "We also wanted to raise the profile of pharmacy in the community. Pharmacy is an underused part of the primary health care team, " she said. Ms Baldwin described the scheme as a sensible way to reach people to promote healthy lifestyles. Paul Heeks, one of the pharmacists taking part in a Boots store in Stowmarket, said that the response from most people had been positive. "It is good that pharmacists are seen to be interested in people's health, " he told The Journal."In the future we want these children to be fit and healthy adults rather than a cost to the NHS." Information packs, which contain practical tips for parents -- for example, how to incorporate healthy snacks into school lunch boxes -- have been well received, Mr Heeks added. Children can choose between a free ball, flying disk, hula hoop or skipping rope after they have been measured. Children found to be overweight are referred to appropriate services offered by the PCT and mescaline, for example, reactions to macrobid.

EDUCATIONAL OBJECTIVE: At the conclusion of this presentation, the participants should be able to develop a rational treatment approach for patients with distant metastatic disease to the thyroid gland from head and neck squamous cell carcinoma. OBJECTIVES: Thyroid gland metastasis from head and neck squamous cell carcinoma SCCA ; is a rare clinical entity. The impact of thyroid metastasis and therapeutic approach to treating these patients is not clearly established. STUDY DESIGN: Retrospective chart review. METHODS: Three patients treated from January 1, 1999, to July 1, 2005, for a primary head and neck SCCA with histopathologically proven metastasis to the thyroid gland were identified. The medical records were reviewed to obtain information regarding demographics, clinical presentation, histology, imaging data, treatment, and survival. RESULTS: The median age of the identified patients was 64 years old. All patients presented with oropharyngeal carcinomas with advanced nodal disease. At the time of diagnosis, all patients were found to have solitary distant metastatic disease to the thyroid gland. These metastases were identified as incidental findings on preoperative imaging studies, and all patients were clinically asymptomatic at the thyroid site. All patients were treated with curative intent using multimodality therapy. As part of the treatment plan, thyroid surgery was performed in all 3 patients with 2 patients undergoing total thyroidectomy and the other having a hemithyroidectomy. No patients experienced recurrent disease in the thyroid region. One patient subsequently developed additional distant metastatic disease, whereas others have not developed further distant metastases with follow up of 6 months and 5 years. CONCLUSIONS: Patients with solitary distant metastatic disease to the thyroid gland from head and neck SCCA can obtain control of disease with multimodality therapy. Thyroidectomy is an effective treatment to control the disease at this metastatic site. 54. Approximately four times more geriatric patients resided in nursing homes before admission, or were discharged to a nursing home after hospitalization, compared with medical patients, and almost twice as many patients admitted to the geriatric ward died during the hospital stay. The main reasons for hospital admission for patients of both study groups were diseases of the circulatory system 26.5% in medical and 22.0% in geriatric patients ; , which mainly presented as ischemic heart disease, heart failure or cerebrovascular disease. Additional common reasons for hospitalization to the geriatric ward were injuries to various parts of the body 12.3% ; most often as a result of falls and unspecified symptoms 11.3% ; such as syncope or collapse. The median number of pharmacologically active substances at hospital admission was similar for both groups 5.5 in the medical ward versus 6 in the geriatric ward; p 0.31 ; , as was the exposure prevalence to various drug classes. The drug classes most often prescribed according to the ATC classification in both patient groups were drugs acting on the cardiovascular system diuretics, ACE inhibitors or angiotensin II receptor antagonists and beta-adrenoceptor antagonists ; , drugs to treat disorders of the gastrointestinal tract or metabolic diseases mineral supplements.
