In this manner, the formulation can be administered only once or twice daily for drugs that would otherwise have to be taken more frequently to maintain required blood levels. Library of Medicine ; and the American Hospital Association Chicago ; ]. Although the reference lists of published trials may be used to locate additional trials, these references may be prone to the same selectivity. In addition, there is clear evidence that authors of RCTs do not usually review their results in the context of the total evidence available from other RCTs 5 ; . As part of a regularly updated systematic review of nutritional supplementation trials for hip-fracture aftercare in the elderly 6 ; , we searched for published and unpublished RCTs. We therefore needed to evaluate our search plan for identifying such nutrition trials. We also wanted to examine whether our strategy could provide the basis for constructing searches for other nutrition-related systematic reviews. Inspection of the 14 systematic reviews included in The Cochrane Library 2000, issue 1; Update Software, Oxford, United Kingdom ; that assessed any form of vitamin supplementation showed that all of the reviewers used MEDLINE, 5 used EMBASE, 1 used BIOSIS, and none used the abstracting service provided by CABNAR. We thus decided to assess the utility of searching different databases, including CABNAR, of searching selected journals by hand including conference proceedings ; , and of contacting authors and experts in the field, because cholinesterase inhibitor.

The AWP System 132. There are approximately 65, 000 different drug products in the United States. Objective: Alzheimer's disease pathology includes both histologic changes and neurotransmitter deficits. The cholinergic deficit contributes to both cognitive and behavioral disturbances, and cholinesterase inhibitors may improve behavior in Alzheimer's disease patients. This analysis was conducted to assess the impact of galantamine, a cholinesterase inhibitor with nicotinic-receptor-modulating properties, on the pattern and evolution of behavioral disturbances in patients with Alzheimer's disease and on caregiver distress related to patients' behavior. Method: Data from 978 patients with mild to moderate Alzheimer's disease who were randomly assigned to placebo or galantamine 8, 16, or 24 mg day ; were analyzed. Behavioral changes were assessed with the Neuropsychiatric Inventory, and alterations in caregiver distress were measured by the Neuropsychiatric Inventory distress scale. Data collected at baseline and 12 and 21 weeks postbaseline were analyzed. Results: N euro ps ych iatric Inve nto ry scores worsened with placebo, whereas patients treated with 16 or 24 mg day of galantamine had no change in total Neurospcyhiatric Inventory scores. Treated patients, asymptomatic or symptomatic at baseline, had better Neuropsychiatric Inventory subscale scores than did patients receiving placebo. Behavioral improvement in patients symptomatic at baseline ranged from 29% to 48%. Changes were evident in patients receiving 16 or 24 mg day of galantamine. High-dose galantamine was associated with a significant reduction in caregiver distress. Conclusions: Galantzmine therapy was associated with reduced emergence of behavioral disturbances and improvement in existing behavioral problems in patients with mild to moderate Alzheimer's disease, with a concomitant reduction in reported caregiver distress. Calcineurin inhibitors Cyclosporine is a cyclic, 11 amino acid peptide that was originally isolated as a fungal fermentation broth during the early 1970s, and its story in terms of the impact on clinical transplantation is well known. It became the catalyst moving the field of transplantation from an interesting clinical research activity to its present position as the acknowledged treatment of choice for many forms of end stage organ failure, including renal failure 19 ; . Cyclosporine binds to form complexes with a ubiquitous, abundant intracellular protein called cyclophilin CYP ; , and the CsA-cyclosporin complex is actually the active immunosuppressive drug. CsA-CYP complexes engage with the calcium dependent serine phosphatase, calcineurin to inhibit enzymatic function. Calcineurin inhibition prevents or reduces activation of a number of important transcription factors, including the nuclear factor of activated T cells NFAT ; . Consequently, CsA action prevents the transcription of a many T cell cytokine genes including IL-2, IFN-G, IL4, TNFa, and GM-CSF. In clinical practice, CsA action results in reduction, but not complete elimination of calcineurin inhibitors CN activity, and the result is immunosuppression leaving sufficient reserve to prevent infection in most circumstances 20 ; . In summary, although clinical use has at times proved challenging, accumulating evidence suggests that sirolimus represents an important advance in clinical immunosuppression. The category of vascular dementia is heterogeneous, and the natural history of the disorder is not well understood 112 ; . Some persons who meet criteria for vascular dementia exhibit a progressive functional decline similar to that seen in Alzheimer disease. Although antihypertensive treatment reduces the development of stroke and dementia, the evidence is limited that a similar course of treatment for persons with mild to moderate vascular dementia delays progression of the disease 51 ; . One systematic review found that aspirin had no effect on cognitive symptoms in persons with vascular dementia 52 ; . One RCT found that nimodipine a calcium-channel blocker ; had no effect on the cognitive and global symptoms of patients with vascular dementia 51 ; , and 1 RCT found that gwlantamine at least stabilized CIBIC score and delayed cognitive deterioration among patients with vascular dementia as well as patients with Alzheimer disease and cerebrovascular disease 113. 2. Patients with non-NSAID non-HP ulcer bleeding had a substantial risk of recurrent bleeding. MANAGEMENT OF GASTROESOPHAGEAL VARICEAL BLEEDING - CRITICAL REVIEW AND LOCAL PERSPECTIVE Dr Hung Hiu Gong, Department of Medicine, North District Hospital December 2003 Gastroenterology and Hepatology Exit Assessment Exercise ; Gastroesophageal variceal bleeding is the most frequent severe complication of portal hypertension and a leading cause of death in patients with cirrhosis. Current literature indicates that the mortality rate of each variceal bleeding episode is 30-50%, and as many as 70 percent of survivors have recurrent bleeding after a first variceal hemorrhage. However, over the last decade, there have been significant developments in the management of portal hypertension and variceal bleeding. The patient outcomes after an episode of acute bleeding indeed are improving. However, how the results of randomized controlled trials apply to clinical care, to what extent they affect the pattern of acute bleeding management and subsequent care, and what about the clinical outcomes outside the clinical trials are largely unknown. In this dissertation, a retrospective study was conducted in a district hospital NDH ; to define the outcomes of variceal bleeding, the risk factors for rebleeding and mortality, and the patterns of practice in the management of variceal bleeding will also be described. Current literatures and guidelines on variceal bleeding management will be critically reviewed. Study methods All patients with documented acute variceal bleeding hospitalized to North District Hospital NDH ; from June 98 to June 2003 were included. The medical records of eligible patients were reviewed to obtain necessary demographic, clinical, endoscopic and follow up data. Patients were followed until death or study closure data, on 30 June 2003. Results A total of 97 patients were included in the study. Upper endoscopy was done in 96.9% of patient within 24 hours with haemostasis rate of 89.7%. Antibiotics and octreotide were given in 42.3% and 74.2% of patients respectively. The inpatient rebleeding and total rebleeding during follow up were 21.6 % and 36.1 % respectively. The inpatient mortality, 6 weeks mortality and total mortality were 19.6%, 24.7% and 43.3% respectively. The predictors of total rebleeding were failed endoscopy haemostasis and follow up by surgical team after acute bleeding. Child's score of B and C at admission and lack of prophylactic treatments were predictors of total mortality. Conclusion This study suggests that there has been a significant reduction in bleeding related mortality in patients with portal hypertension. The advance in therapeutic modalities for the management of acute variceal bleeding and subsequent care may offer an explanation for the improved survival.
