By filing multiple claims in a single race owners and trainers are colluding to improve their chances of either preventing a horse from being claimed or successfully claiming a horse against other bidders. Results Inhibitory Effects of the Ethyl Acetate Extracts of Citrus Juices on the Activity of CYP3A4 and Metabolism of Tacrolimus. Linear dependence of renal clearance on drug free fraction was demonstrated in IPK studies with furosemide [51], diggoxin [23], and chlorothiazide [52]. While these results are consistent with extraction ratio theory, IPK investigations with sulfisoxazole [12] and acetazolamide [53] demonstrated evidence for tubular secretion of bound drug. In the acetazolamide study, the results were attributed to a facilitative role of albumin in renal tubular transport. Whereas albumin is the primary binding protein in plasma, complexation with other binding proteins can also influence drug disposition. The IPK was utilized to study the effects of lipoprotein HDL and LDL ; on the renal disposition of cyclosporine [54]. The presence of lipoprotein significantly reduced cyclosporine renal clearance and kidney accumulation. The study further illustrated the versatility of the IPK technique by allowing the inclusion of other proteins in the recirculating perfusate. Renal Drug Metabolism Although the liver is the responsible for drug biotransformation, the kidney contains many drug-metabolizing enzymes, and contributes to renal metabolism of a number of medications [55-57]. Biotransformation e.g. glucuronidation, acetylation ; of a number of medications has been demonstrated in the IPK including ibuprofen, prednisolone, sulfisoxazole and glutathione [12, 58-61]. One distinct advantage of the IPK over other in vitro methods is that the overall contributions of transport mechanisms and biotransformation by the kidney can be elucidated. Additionally, mechanisms of excretion of formed metabolites can also be tracked in the IPK. Following bolus administration of quinapril an angiotensin convertingenzyme inhibitor ; in the IPK, less than 0.1% of the dose was recovered in the urine [21]. While this suggested that extensive tubular reabsorption of the drug, greater than 99% of the dose was intrarenally converted to the metabolite quinaliprat in the kidney cell. Overall, the study demonstrated that quinapril is actually rapidly secreted into the kidney cell, but that this process is masked by extensive biotransformation. The perfused kidney model has also been used to study the renal disposition of salicylic acid and salicyuric acid [10, 11, 40, 62, Salicylic acid SA ; undergoes glycine conjugation to form salicyluric acid SU ; , but this reaction was reversible in the IPK. Significant differences in SA excretion were noted between experiments, where SA was directly added to the IPK and those where SA was tracked as a metabolite following SU administration. SA clearance was dramatically higher as a formed metabolite, suggesting that basolateral transport is the rate-limiting step to excretion. Additionally, dose-escalation studies demonstrated reduced formation of metabolite at higher doses of drug. Drug-Drug Interactions Interaction experiments are routinely utilized in IPK studies. There are two reasons for carrying out these studies: 1 ; to determine confirm pathways contributing to drug excretion; and 2 ; to identify medications that may display clinically significant interactions when co-administered to patients.

Patients treated with digoxin for heart failure did not derive any overall mortality benefit from treatment, but did have reduced hospitalizations. Additionally, older patients are more likely to have impaired renal function and as such be prone to digoxin toxicity. As a result, it is sensible to monitor the dose of digoxin, carefully utilizing as low a dose as possible, with monitoring digoxin levels in the plasma as an important consideration. Anticoagulation should be considered in all patients who have heart failure and atrial fibrillation. Careful consideration is required before treatment commences when patients are frail and have associated cognitive impairment.
