Epidemiologic studies have suggested that DPN is very common. Dyck and colleagues, in a communitywide cross-sectional study in Rochester, Minnesota, found that 66% of patients with type 1 diabetes had some form of neuropathy, and that 55% had DPN. In patients with types 2 diabetes, 59% had some form of neuropathy, and 45% had DPN.12 It should also be noted that the reported prevalence of DPN will depend to some extent on how the neuropathy is defined: studies in which detailed electrophysiological tests are performed may report higher prevalence rates than studies that report only symptomatic neuropathy. DPN ultimately results in the loss of both largeand small-diameter fibers, usually beginning with the smaller fibers. Vascular changes, such as occluded vessels, shunting, and dilated capillaries, accompany the progressive loss of nerve fibers. As DPN advances, the axons atrophy and are lost, and the nerve is depopulated. This loss of nerve fibers is responsible for the manifestations of DPN, including ataxia, erectile dysfunction, abnormal position sense, and abnormal deep tendon reflexes, as well as other microvascular compli and ddavp.

'When I saw [Mr T] the following morning 18.12.98 ; he was in agony because his analgesia had been stopped. I got further results on his blood cultures which showed the organism was the more aggressive Staphylococcus aureus rather than the Staph[ylococcus] epidermidis I had been expecting. An urgent chest X-ray was carried out which showed widespread nodular shadowing in both lung fields consistent with multiple septic emboli from the Hickman line. I had already contacted [a consultant anaesthetist and pain specialist] to review [Mr T's] analgesia so he removed [Mr T's] line on the ward and set up a peripheral line. I changed [Mr T's] antibiotic from Teicoplanin to Flucloxacillin. I hoped that, with the focus of infection removed, he would rapidly respond. I wrote instructions to contact the consultant haematologist on call for the weekend if the situation deteriorated. 'Unfortunately, . [M r T] continued to deteriorate and developed septic shock later that evening . 'It is easy to see what went wrong in hindsight, but at the time the clinical picture was far from clear. [Mr T's] severe pain and narcotic analgesics were a major hindrance to diagnosing the infection as he was alternately comatose and sweating with pain depending on his levels of analgesia. His lack of fever due to recent steroid therapy, perhaps? ; and normal neutrophil levels also provided false reassurance ' and desmopressin.

Its name is often applied to this category of drugs. Oxacillin, cloxacillin, and dicloxacillin were subsequently developed for oral use distasteful ; . Nafcillin is for parenteral use. These agents are more effective than methicillin against staphylococci, but none of them offers any advantages over penicillin vs. streptococci or pneumococci. Oral administration should be in the fasting state, and dicloxacillin achieves the highest serum levels. Prolonged over 21 days ; therapy with nafcillin may cause a reversible neutropenia. The chronometric absolute magnitudes show no trend with redshift and a moderate dispersion, quite comparable to that for optical absolute magnitudes and distinctly less than that in the observed apparent magnitudes, despite the observational magnitude cutoff. A summary of the results of statistical analysis Table 1 ; shows that this is the case both overall and in all substantial redshift regions. The bright envelope Since a distance-dependent and hence presumably redshiftdependent ; observational magnitude cutoff can produce a trend in absolute magnitudes with redshift and can less readily increase dispersion beyond that in apparent magnitudes, the deviations from predictions for an unbiased sample do not necessarily derogate the theory rigorously. The question is one of the size of the bias or its limitation by suitable treatment of the data. In the latter respect, the classic and direct method of formation of the bright envelope is useful.
